Ooh! I noticed the color of the liquid in the syringe was a gold-ish color! So maybe it's representing someone putting gold inside of his head? Mitzi getting the injection too probably represents the relationship between her and Beach Bear!
If that's not the answer could it possibly represent genetic mutation and contol by the government in a time of crysis in the future? (If you've ever read "The Giver" you'll see what I mean.)
